How do I make a referral to palliative care?

We are passionate about providing the best possible care for people with a progressive life-limiting illness delivered by our multidisciplinary team of healthcare professionals and volunteers focusing on the individual , their family, and carers

All patient referrals must be from a General Practitioner, medical specialist, or advance practice nurse, and include:

  • the referring practitioner’s address
  • phone number
  • provider number.

Please address your letter of referral to our palliative specialists. If the referral is from a specialist, the patient’s General Practicioner details should also be included with the referral. Remember: your referral must be legally valid as outlined in the Medicare Benefits Schedule (MBS). It must also be received and dated before your patient starts their care.
