Together with expertise in clinical and dementia care, Caulfield Village is designed to help you feel familiar in your environment and make your own decisions.
As well as an onsite general store and salon, each cottage has easy access to gardens, a central domestic kitchen, and a mix of shared living areas and quiet comfortable spots to sit and relax.

As an approved provider of the Specialist Dementia Care Program (SDCP) in Melbourne, Caulfield Village provides individualised residential aged care for people experiencing very severe behaviours and psychological symptoms of dementia (BPSD).
The SDCP aims to stabilise and reduce a person’s behavioural symptoms so they can ultimately transition to a less intensive care setting.

The Glens is a double-storey care home with five living zones for older people with complex health needs.
Each area caters for up to 15 people with similar needs, which promotes personalised care within a socially inclusive environment.
Our team is qualified in specialised care and backed by HammondCare’s history and expertise, encompassing over 90 years caring for people in need.
In partnership with Alfred Health at Caulfield Hospital, The Glens delivers the Transition Care Program (TCP) for people in Melbourne who need more time to recover following a hospital stay.
The TCP supports people to improve their physical, cognitive and psychosocial health through access to a range of care services in a non-hospital setting.
The primary goal of the TCP is to enable people to continue living independently. Alternatively, the program supports people to optimise their health while assisting them and their families/carers to make appropriate long-term care arrangements.
The TCP is jointly funded by the Commonwealth and state/territory governments.
