How to refer to HammondCare healthcare
Our referral processes may vary slightly depending on the healthcare location and services for which you are seeking care.

Ready to make a referral?
Palliative and supportive care
Greenwich and Neringah Hospitals; Northern Sydney Community Palliative Care Service; Braeside Hospital
All patient referrals must be from a General Practitioner, medical specialist, or advance practice nurse, and must include the referring officer’s address, phone number, and provider number. If the referral is from a specialist, the patient’s GP details should also be included with the referral.
All referrals must be legally valid as outlined in the Medicare Benefits Schedule (MBS).
The referral must be received and dated prior to the patient receiving treatment from our service, with the exception of medical emergencies and inter-hospital transfers.

- Complete the inpatient and supportive care referral form
Northern Sydney Community Palliative Care Service referrals
- Check community palliative care admission criteria
- Complete the community palliative and supportive care form
Braeside Hospital inpatient referrals
- Check inpatient admission criteria
- Complete the inpatient palliative care referral form

Rehabilitation
Greenwich and Braeside Hospitals

Greenwich Hospital
Please complete the form/s below:
- For Ambulatory rehabilitation: Ambulatory Rehabilitation Services Referral Form
- For hydrotherapy: Hydrotherapy Referral / Medical Clearance Form
Braeside Hospital
- Check the rehabilitation admission criteria
- Complete the rehabilitation outpatient service form
And email or fax as per instructions on the form.
